First Pass Metabolism

Medications that are taken orally are absorbed through the stomach.   These medications enter the “portal circulation” and are transported to the liver.  Metabolism within the smooth endoplasmic reticulum occurs primarily in the liver. Liver enzymes then breakdown most of the medication molecules, leaving only a small portion of the medication to enter central circulation.
