On Scene and En Route Care of Stroke Patients

Caveats for care en route include:

  • Do not treat hypertension, but do monitor blood pressure en route.
  • Do not administer dextrose containing fluids to patients who are not hypoglycemic.
  • Avoid excess IV fluid administration.
  • Keep oxygen saturation above 94% by use of supplemental oxygen.
  • Assist ventilation, if required.
  • Position patient in a comfortable position.
  • Protect the airway and be aware of potential for aspiration.
  • Follow state or local policy on diversion to the nearest stroke center. [69]
