Criteria for IV rt-PA (47)

Inclusion criteria
Acute ischemic stroke
Age 18-80 y.o.
Onset of stroke symptoms 3-4.5 hours before rt-PA administration
Stroke symptoms present for at least 30 min without significant improvement

Exclusion criteria
Intracranial hemorrhage
Symptom onset unknown
Rapidly improving symptoms
Severe symptoms (NIHSS>25)
Seizure at stroke onset
Stroke or serious head trauma in last 3 months
Prior stroke and diabetes
Heparin administration in preceding 48 hours
Platelets <100,000/mm3
BP >185/110 or if aggressive IV medications required to reduce to these limits
Blood glucose <50 or >400 mg/dL
Oral anticoagulant use
Major surgery or severe head trauma in last 3 months
Other major disorder that increases risk of bleeding
