Documentation of Restraint

EMS personnel must document what actions on the part of the patient forced the care provider to choose or escalate to each progressive intervention level.  In addition, it may be beneficial to include “Patient was restrained for the safety of the patient and EMS” within your PCR as a means of using justified force to restrain someone.  If physical restraints were used on extremities, the documentation of pulses, motor function and sensory response must also be documented for each extremity that has been restrained.
