Tracheostomy Tube Management for BLS Providers

Procedure for field change of tracheostomy tube 

  1. Put on the appropriate body substance isolation. 
  2. Open a sterile burn sheet as your sterile field 
  3. Open sterile supplies and place on sterile field 
  4. Put dominant hand into a sterile glove. 
  5. Check the cuff: insert 10 cc of air in the cuff and check for leaks. 
  6. Deflate the cuff of the new trach tube and do not contaminate the tube by touching anything. 
  7. Remove the inner cannula and replace with the obturator. 
  8. Lubricate the tube tip with a water-soluble sterile lubricant. Do not use any petroleum-based products. 
  9. Suction patient as needed. 
  10. If the patient has a cuff, deflate it by pulling all air out with syringe. 
  11. Remove the trach dressing and holder. 
  12. Wipe the neck to clean skin as needed with sterile 4 x 4 and sterile water. 
  13. If the patient is connected to a ventilator, disconnect patient from the ventilator. 
  14. Remove the tracheostomy tube using a forward and posterior motion. 
  15. Using your sterile hand, carefully insert the new tube with the obturator in place. 
  16. Remove the obturator immediately. 
  17. Insert the inner cannula into the tracheostomy tube. 
  18. Inflate the cuff using minimal leak technique or a cuff manometer. 
  19. Ventilate the patient using the bag-valve-mask; looking for chest rise. 
  20. Attach waveform capnography.  (ALS ONLY)
  21. With the tube in place, stabilize it with one hand and secure with a tube holder. 
  22. Continue to assist ventilations as necessary.
