In order to reinsert the tracheostomy tube, follow the procedure outlined below:
- Insert the obturator and put a water-soluble lubricant on the outer cannula.
- Remember a smaller sized tracheal tube may be utilized as trauma may have caused swelling of the tract
- Insert the tracheostomy tube while using a upward curved motion, and while applying gentle pressure, the tube will slide into place.
- With the tube in place, remove the obturator immediately.
- Inflate the tracheostomy cuff.
- Attempt ventilation and look for signs of proper / improper placement.
- Visible chest rise
- Capnography waveform (ALS ONLY)
- Apply the trach tube holder to stabilize the tracheostomy tube.
- Insert the inner cannula.
- Begin providing assisted ventilations and a continuous assessment of the patient noting patient condition. (1)
Despite the outcome of the tracheostomy change the patient should be transported to the hospital for further evaluation and x-ray confirmation of proper placement. | | | | |
