Psychological Considerations
Depression is common among patients on prolonged mechanical ventilation. One study of 336 patients in a long-term acute care facility revealed depressive disorders among 42 percent. A study of 41 patients transferred to a long-term acute care facility for prolonged mechanical ventilation revealed a diagnosis of post-traumatic stress disorder within three months of weaning. (22)

Because psychological factors contribute to ventilator dependence, stress should be minimized in all possible ways. With respect to family, caregivers and EMTs this should include frequent communication. Depression can be mitigated somewhat by ambulation and stimulation through the environment, using television, books, radio, and visits with significant people in the patient’s life. (23) Biofeedback techniques may alleviate some difficulties in weaning, if the patient meets parameters for weaning from mechanical ventilation. Biofeedback and hypnosis can be effective techniques to reduce stress. (24)
