On ultrasound, when the hCG level reaches 1500-2000 mIU /ml, transvaginal ultrasound will show a gestational sac, and when the gestational sac reaches 10 mm, there should be a yolk sac seen on ultrasound. The beta subunit of hCG should increase by 80% every 48 hours. Any failure to reach these milestones may indicate failure of the early pregnancy. Failure of early pregnancy can be managed with misoprostol, unless gestational trophoblastic disease is present.
