If the mother is less than 24 weeks gestation with severe preeclampsia, admission is indicated, and maintenance of blood pressure should be less than 155/90, with diastolic pressures of less than 90, by using antihypertensive medications. ACE inhibitors or angiotension receptor blockers (ARBs) should not be used because they have teratogenic effects. Labetolol is frequently used.
Magnesium sulfate (MgSO4) should be given IV to prevent seizure, the health of mother and fetus should be closely monitored, and delivery should be induced as soon as the fetus is adequately developed. |
